This website assists with the management of the Western Sydney HealthPathways program. The site includes material such as planning documents, minutes and agendas, details on key events, and status updates on pathways in progress and local resource development.

Western Sydney HealthPathways provides web-based information on management and referral pathways to help clinicians navigate patients through the complex primary, community, and acute care system within the Western Sydney region. HealthPathways is designed to be used at the point of care, primarily for general practice but is also available to specialists, nurses, allied health and other health professionals.

Important Note about Western Sydney HealthPathways

The HealthPathways portal is not designed for patient use, therefore a username and password is only provided to health professionals. However, many of the pathways do contain patient information which can be printed off and given to patients at the time of consultation.

The pathways that have been localised to the Western Sydney region can be found on the left hand side Table of Contents under ‘Recently Localised Pathways' .

Until all pathways are localised for Western Sydney, information developed in Canterbury, New Zealand will be available. Pathways that have been developed in New Zealand and are not yet localised for Western Sydney have a pale orange background and a map of New Zealand repeated on the right side of the page. New Zealand pathways may contain useful condition management information. However, please use your clinical judgement as to the appropriateness to the Western Sydney region. Please keep this in mind whilst viewing the site.

 

Western Sydney HealthPathways is a collaboration between WentWest, Western Sydney Local Health District and the Sydney Children's Hospitals Network, HealthPathways is an initiative from the Canterbury District Health Board (CDHB) in Christchurch, New Zealand, which provides a ‘whole of system’ approach to effective integration of acute and primary health care services.
